1. A Living Will

This document gives the person you designate the power to make most health care decisions for you if you lose the capacity to make informed health care decisions for yourself. Effective only when your attending physician determines that you have lost the capacity to make informed health care decisions for yourself.
3. A Durable POA for Finances
Through this document you appoint a person to act on your behalf on a variety of matters such as banking, investing, real estate, tax returns, gifting, amending documents, signing your name, etc. Effective when you have lost the capacity to make such decisions.
4. A Last Will and Testament
Through this document you appoint a person to pay your final debts and expenses and distribute your assets upon your death to those individuals or organizations you have chosen.
5. A Listing of Assets with Financial Information
This makes the probate or trust process much easier on all parties involved by preparing in advance a complete Financial Statement.
